- If either party is not Catholic, this form is used to provide documentation if that person had ever been married before.

- When planning your ceremony, you can pick up a "Together for Life" book at the parish office.
- Then download the appropriate form below depending on if your wedding will include a mass.
- Once you have filled out the form, return it along with the "Together for Life" book to the Priest/Deacon preparing your paperwork.
